Odontogenic Maxillary Sinusitis

What is it?

Odontogenic maxillary sinusitis is a condition characterised by inflammation and infection of the maxillary sinus, which is the hollow space located inside the maxillary bone (upper jaw). It occurs when the roots of upper teeth, particularly molars or premolars, are too close to the sinus and become infected or develop a dental abscess. The infection can spread from the dental structures to the maxillary sinus, leading to sinusitis.

Causes and Symptoms

Odontogenic maxillary sinusitis is usually caused by dental problems, such as untreated tooth decay, dental abscesses, missed dental veneers or dental trauma. Infection of the maxillary sinus can be aggravated by obstruction of the small drainage channels called sinus ostia. Common symptoms of odontogenic maxillary sinusitis may include:

  1. Facial pain or pressure, often localised to the area above the affected tooth or sinus.
  2. Toothache in the upper jaw, especially in the back teeth.
  3. Sinus congestion or a feeling of fullness on the affected side of the face.
  4. Foul-smelling breath (halitosis).
  5. Drainage of pus from the nose or throat.
  6. Headache.
  7. Fever (in severe cases).

Diagnosis

Diagnosis of odontogenic maxillary sinusitis usually involves a thorough evaluation by an oral and facial surgeon or an otolaryngologist. Diagnosis may include:

  1. Clinical examination: The health professional will examine your teeth, gums and the inside of your mouth for signs of infection or dental problems.
  2. Imaging: Radiographs, dental cone beam computed tomography (CBCT) or imaging of the maxillary sinus (such as x-rays or CT scans of the maxillary sinus) may be performed to assess the relationship between dental structures and the maxillary sinus and to identify any signs of infection or inflammation.
  3. Nasal endoscopy: A nasal endoscopy may be performed to directly visualise the nasal and sinus passages and identify any signs of infection or obstruction.

Treatment

Treatment of odontogenic maxillary sinusitis aims to clear the infection, relieve symptoms and prevent recurrence. Treatment options may include:

  1. Relief of symptoms: Analgesics, nasal decongestants, saline nasal rinses and warm compresses may be recommended to provide symptomatic relief and aid the healing process.
  2. Antibiotics: Depending on the severity of the infection, your healthcare professional may prescribe antibiotics to help clear the infection. Antibiotics may be used in conjunction with surgery or as a stand-alone treatment option.
  3. Oral surgery: If the infection is caused by a dental problem, such as a tooth abscess or a failed root canal, oral surgery may be recommended. The procedure involves treating the dental problem, such as performing an apicoectomy, extracting the affected tooth or draining the dental abscess.
  4. Endoscopic sinus surgery: In some cases, if the infection is severe or there is obstruction of the sinus, endoscopic sinus surgery may be necessary. This procedure involves using a thin, flexible tube with a camera (endoscope) to access the maxillary sinus through the nose. The surgeon can remove any obstructions, clean the sinus cavity and create drainage pathways to prevent future infections.
